California Trim Rx LLC may use your health information for conducting our business. Examples: Treatment – We will use your health information to appropriately determine your qualification for our program. For
example, we will review your intake forms and medical information with our doctor to determine if you qualify for treatment. We may also use your PHI (Personal Health Information) to give you helpful information such as program updates.
YOUR INDIVIDUAL RIGHTS
You have the right to request in writing restrictions on how we use and share your health information. We consider all requests for restrictions carefully but are not required to agree to any restriction. You may request that we use a specific telephone number or address to communicate with you or that we may leave a message (or not). You may request to inspect and get a copy of your health information (including an electronic copy if we maintain the record electronically). Fees may apply. You have the right to request confidential communications, restrict disclosures, access and amend records, receive an accounting of disclosures, and obtain a paper copy of the notice. SHARING YOUR HEALTH INFORMATION

These situations include activities necessary to provide you with our weight loss product or to our business associates that perform services on our behalf.
